All Youth Participants are required to have a parental activity and medical release submitted for the conference.
Please download and complete the form. The signed form needs to be uploaded to the website for each participant.
All participants must have a signed form to participate in youth conference. If you do not have a scanner, please use a smartphone, tablet or camera to
take a readable photo of the form and upload the image to the website. If you are having difficulty, please contact your ward or stake youth conference
specialist to assist you.
